ABSTRACT

ABSTRACT Fibroepithelial polyps are benign non-epithelial tumors arising from mesodermal tissue and are commonly found on the skin. They can develop on the eyelid and surrounding area and rarely appear at other sites. While most cutaneous cases are diagnosed incidentally, the lesions may be symptomatic based on their location. The etiology of fibroepithelial polyps is not fully understood but may involve trauma, chronic irritation, allergic factors, and developmental or congenital causes. Although fibroepithelial polyps are benign lesions, they can be malignant in extremely rare cases. Herein we discuss 2 cases of fibroepithelial polyp with an atypical presentation and their treatment.


RESUMO Pólipos fibroepiteliais são tumores benignos não-epiteliais que surgem do tecido mesodérmico e são comumente encontrados na pele. Eles podem se desenvolver na pálpebra e na área adjacente, e raramente aparecem em outros locais. Embora a maioria dos casos cutâneos seja diagnosticada incidentalmente, as lesões podem ser sintomáticas com base em sua localização. A etiologia dos pólipos fibroepiteliais não é completamente compreendida, mas pode envolver trauma, irritação crônica, fatores alérgicos e causas de desenvolvimento ou congênitas. Apesar dos pólipos fibroepiteliais serem lesões benignas, podem ser malignos em casos extremamente raros. Aqui discutimos 2 casos de pólipo fibroepitelial com uma apresentação atípica e seu tratamento.

ABSTRACT

ABSTRACT In this systematic review, we evaluated studies involving adjuvant and primary topical treatment for ocular surface squamous neoplasia (OSSN). The findings were: (i) adjuvant 5-fluorouracil (5-FU) reduces the risk of relapse after surgical excision with mild side effects [level Ib, grade of recommendation (GR) A]. (ii) Primary topical mitomycin (MMC) produces a high rate of complete response, low recurrence rate, and mild side effects (level Ib, GR A). (iii) Primary chemotherapy versus adjuvant chemotherapy produce similar rates of recurrence, with no significant difference (level IIb, GR B). (iv) Adjuvant 5-FU versus MMC showed no significant differences, with mild side effects in both groups and a better toxicity profile for MMC (level III, GR C). (v) Primary topical 5-FU versus MMC versus interferon (IFN) showed similar rates of tumor recurrence, mild side effects for all drugs, and more severe side effects in the 5-FU arm, followed successively by MMC and IFN (level III, GR C).


RESUMO Revisão sistemática envolvendo estudos sobre o tratamento adjuvante e tratamento tópico primário para a neoplasia escamosa da superfície ocular. Os resultados foram: (i) 5-fluorouracil adjuvante reduziu o risco de recidiva após a excisão cirúrgica com efeitos colaterais leves (nível Ib, Grau de recomendação (GR) A). (ii) Mitomicina tópica primária produziu uma alta taxa de resposta completa, baixa taxa de recorrência e efeitos colaterais leves (nível Ib, GR A). (iii) Quimioterapia primária versus adjuvante produz taxas semelhantes de recorrência (nível IIb, GR B). (iv) 5- 5-FU versus mitomicina adjuvante não mostrou diferenças significativas nas taxas de recorrencia, com efeitos coalterais leves em ambos os grupos e melhor perfil de toxicidade para mitomicina (nível III, GR C). (v) 5- 5-FU tópico primário versus mitomicina ou interferon (INF) apresentam taxa similar de recorrência, com efeito colateral leve, mas com maior incidencia no braço 5- 5-FU, seguido pela Mitomicina e IFN (nível III, GR C).

ABSTRACT

RESUMO Objetivo: Avaliar a segurança e eficácia de três técnicas distintas para o tratamento de neoplasia intraepitelial córneo-conjuntival. Métodos: Vinte e seis pacientes, 11 mulheres e 15 homens, com idade entre 32 e 88 anos (média 64,84 anos), atendidos entre 1999 e 2014, foram incluídos neste estudo. Todos os pacientes foram submetidos à exérese da lesão com margem de segurança de 4mm e crioterapia a 2mm do limbo. Conforme o tratamento complementar, os pacientes foram divididos em três grupos: Grupo 1 (8 olhos): recobrimento conjuntival e 2 ciclos de mitomicina C 0,02% (MMC) 4x/dia por 10 dias, com intervalo de 30 dias entre os ciclos. Grupo 2 (9 olhos): recobrimento com membrana amniótica e 3 ciclos de MMC 3x/dia por 10 dias, com intervalo de 10 dias entre o 1º e o 2º ciclo e 30 dias entre 2º e o 3º ciclo. Grupo 3 (9 olhos): recobrimento com membrana amniótica sem o uso de MMC. O acompanhamento dos pacientes variou de 3 meses a 14 anos (média: 4 anos). Resultados: Vinte e três pacientes apresentaram erradicação da lesão com um único tratamento. Três pacientes do Grupo 3 apresentaram recidiva da lesão após 3 meses, 6 meses e 1 ano do tratamento. Dois deles foram retratados pela técnica 1, sem nova recidiva após seguimento de 3 e 4 anos; o outro paciente foi submetido à enucleação. Não foram observados efeitos colaterais relacionados ao uso da mitomicina C durante o seguimento. Conclusão: Após exérese da lesão e crioterapia, os pacientes tratados com recobrimento conjuntival ou membrana amniótica associado à mitomicina C mostraram tendência a resultados mais eficazes.


ABSTRACT Purpose: To evaluate the safety and efficacy of three different techniques for the treatment of conjunctival-corneal intraepithelial neoplasia. Methods: Twenty-six eyes of 26 patients, 11 women and 15 men were included in a nonrandomized, retrospective, observational case series. Mean patient age was 64 years (range, 32-88 years).All patients were treated from 1999 to 2014. Mean postoperative follow-up was 4 years (range, 3 months to 14 years). All patients underwent excision of the lesion with a 4-mm safety margin and a 2-mm from the limbus cryotherapy. As further treatment the patients were divided into three groups: Group 1 (8 eyes): conjunctival autograft and 2 cycles of mitomycin C 0.02% (MMC) eye drops, four times daily for 10 days with an interval of 30 days between cycles. Group 2 (9 eyes): amniotic membrane (AM) graft and 3 cycles of MMC eye drops, three times daily for 10 days with an interval of 10 days between the 1st and 2nd cycle and 30 days between the 2nd and 3rd cycle. Group 3 (9 eyes): AM graft without MMC eye drops. Results: After a single treatment, the lesions were eradicated in twenty-three patients. Recurrence was noted in three patients from Group 3 at 3 months, 6 months and 1 year after treatment. Two of them were retreated by the technique used in Group 1 without further recurrence; the other patient underwent enucleation. No adverse effects related to the use of mitomycin C were observed during. There were no side effects related to the use of mitomycin C during follow-up. Conclusion: After surgical excision and cryotherapy, patients treated with conjunctival or amniotic membrane graft associated with mitomycin C, showed a tendency to more effective results.

ABSTRACT

OBJETIVO: describir las alteraciones morfológicas y morfométricas mediante microscopia confocal en el tejido corneal expuesto al aceite de silicona en pacientes vitrectomizados antes y después de la cirugía de catarata. MÉTODOS: estudio prospectivo, descriptivo, longitudinal y observacional en 34 ojos de pacientes operados de catarata por facoemulsificación con el uso de la técnica de facochop por córnea clara, previamente vitrectomizados con aceite de silicona de 1 000 centistokes en cavidad vítrea que presentaban aceite emulsionado en cámara anterior, a los cuales se les realizó microscopia confocal previa y posteriormente a la cirugía de catarata con microscopio confocal CONFOSCAN S-4/NIDEK de cuarta generación en el período comprendido entre febrero y diciembre de 2012. RESULTADOS: predominó el sexo masculino (64,7 %). El mayor número de casos estuvo en el rango de edades entre 39 y 58 años (47 %). La densidad celular endotelial fue más baja a nivel superior que a nivel central de la córnea que coincide con el área de mayor exposición al aceite de silicona tanto posvitrectomía pars-plana (VPP) como poscirugía de catarata con pérdida celular promedio de 358 células/mm2 (14,7 %). Los valores de pleomorfismo y polimegatismo fueron patológicos después de la cirugía secuencial. Hubo lesiones endoteliales concomitantes en el 100 % de la muestra. CONCLUSIONES: la microscopia confocal es una herramienta novedosa de gran eficacia para la detección y seguimiento de las modificaciones iniciales en el endotelio corneal que permanece en contacto con el aceite de silicona emulsionado en los pacientes vitrectomizados antes de que aparezcan signos clínicos de queratopatía


OBJECTIVE: to describe with the confocal microscopy the morphological and morphometric changes occurred in the corneal tissues exposed to silicone oil in vitrectomized patients before and after cataract surgery. METHODS: a prospective, descriptive, longitudinal and observational study was carried out in 34 eyes of patients operated on for cataract by phacoemulsification using the phacochop technique for clear cornea. These patients had been previously vitrectomized in anterior chamber with 1 000 centistoke emulsified silicone oil and had been performed confocal microscopy with 4th generation confocal microscope CONFOSCAN-S-4 NIDEK before and after cataract surgery in the period of February to December 2012. RESULTS: Males prevailed (64,7 %). The highest number of cases aged 39 to 58 years (47 %). The endothelial cell density was lower at upper level than at central level of the cornea, which agreed with the most exposed area to silicone oil after pars-plana vitrectomy and after cataract surgery. The average cell loss was 358 cell/mm2 (14,7 %). Pleomorphism and polymegatism values were pathological after the sequential surgery. There were concomitant endothelial injuries observed in 100% of cases. CONCLUSIONS: The confocal microscopy is a greatly effective innovative tool for the detection and follow-up of initial changes in the corneal endothelium that remains in contact with the emulsified silicone oil in vitrectomized patients before the onset of clinical signs of keratopathy.

ABSTRACT

OBJETIVO: determinar las características clínicas e histopatológicas de los tumores en los anexos oculares durante el período 2009 al 2010. MÉTODOS: se realizó un estudio observacional descriptivo longitudinal retrospectivo en un grupo de 166 pacientes con diagnóstico de tumor de anexos oculares, escogidos en consulta de Oculoplastia en Holguín. RESULTADOS: predominó la edad comprendida entre 41 a 50 años, con 37,35 %. El sexo más afectado fue el femenino con 53,61 %. La mayor cantidad de lesiones se encontró en los párpados de 114 pacientes. El tipo histológico mayormente identificado fue el nevus con 25,90 %, seguido por el carcinoma basal con 18,07 %. Se obtuvo una concordancia de 83,13 % entre el diagnóstico clínico y el histológico. CONCLUSIONES: en los pacientes estudiados las lesiones tumorales de los anexos se expresan comúnmente en el sexo femenino con una edad riesgosa a partir de la cuarta década de la vida donde los parpados son los más susceptibles. El nevus y el carcinoma basocelular se comportaron como un problema de salud por su predominante frecuencia, con una buena utilidad del método clínico en su diagnóstico.


OBJECTIVE: to describe the clinical and histopathological signs of ocular adnexa tumors detected in the 2009-2010 period. METHODS: retrospective, longitudinal, descriptive and observational study of 166 patients presenting with some adnexa tumors, who were chosen at the oculoplasty service located in Holguin province. RESULTS: the 41 to 50 years-old group accounted for 37,35 %. The most affected patients were females with 53,61 %. The greatest amount of injures was found in the eyelids of 114 individuals; the mostly identified histological type was nevus which represented 25,90 % of cases, followed by basal carcinoma with 18,07 %. An agreement of 83,13 % between the clinical and the histopathological diagnosis was reached. CONCLUSIONS: the tumor lesions of adnexa found in the studied patients commonly occur in females at risky ages as of the 40 years when the eyelids are more sensitive. Nevus and basocelular carcinoma became health problems due to their predominant frequency, and they were correctly diagnosed by the clinical method.

ABSTRACT

Purpose: To describe the cases of conjunctival melanoma (CM) and report the disease-free interval (DFI) and overall survival (OS). Methods: The charts of 22 patients who were admitted to two hospitals between 1985 and 2006 were reviewed for pertinent data, including demographics, site of involvement in the conjunctiva and sub-sites, surgical treatment, and adjuvant treatment. Results: There were 10 (45.45%) males and 12 (54.55%) females. Mean age was 52.3 years. In this group, 15 patients (68.1%) involved the bulbar conjunctiva, and 7 (31.9%) involved the palpebral conjunctiva. Of the 22 patients, 72.72% had a history of conjunctival melanosis. The average tumor size was 20.4 mm. Eight (36.36%) patients underwent orbital exenteration, 2 (9.06%) had enucleation, 5 (22.72%) had wide excision of the lesion followed by radiotherapy, 2 (9.06%) had orbital exenteration with neck dissection, and the remaining 5 patients (22.72%) were considered adequately treated only with wide excision. Eight (36.36%) patients received adjuvant treatment. Disease-free survival at 5 years was 51% and the overall survival at 5 and 10 years was 50% and 37%, respectively. Conclusion: Conjunctival melanoma is a rare entity. Tumor behavior is aggressive, and the optimal treatment is surgery with adjuvant therapy. .


Objetivo: Descrever o intervalo livre de doença (DFI) e sobrevida global (OS) de pacientes com melanoma conjuntival (CM). Método: Prontuários de 22 pacientes que foram internados em dois hospitais entre 1985 e 2006 foram revisados para dados pertinentes, incluindo dados demográficos, local de envolvimento na conjuntiva e outros locais de acometimento, tratamento cirúrgico e tratamento adjuvante. Resultados: Dez (45,45%) homens e 12 (54,55%) mulheres foram selecionados. A média de idade foi de 52,3 anos. Em 15 pacientes (68,1%) CM envolveu a conjuntiva bulbar, e em 7 (31,9%) envolveu a conjuntiva palpebral. Dos 22 pacientes, 72,72% tinham história de melanose conjuntival. O tamanho médio do tumor foi de 20,4 mm. Oito (36,36%) pacientes foram submetidos à exenteração orbital, 2 (9,06% ) à enucleação, 5 (22,72%) à ampla excisão da lesão seguida de radioterapia, 2 (9,06%) à exenteração orbital com esvaziamento cervical e os restantes 5 pacientes (22,72%) foram considerados adequadamente tratados apenas com excisão ampla. Oito (36,36%) pacientes receberam tratamento adjuvante. Sobrevida livre de doença em 5 anos foi de 51% e sobrevida global em 5 e 10 anos foi de 50% e 37%, respectivamente. Conclusão: Melanoma conjuntival é uma entidade rara. Comportamento do tumor é agressivo, e o melhor tratamento é a cirurgia com terapia adjuvante. .

INTRODUÇÃO: Os tumores melanocíticos conjuntivais compreendem lesões que podem variar desde lesões benignas como os nevos conjuntivais, lesões pré-cancerosas como melanose adquirida primária com atipia até o melanoma maligno conjuntival. O reconhecimento das características clinicas destas lesões e seu diagnóstico preciso permitem o tratamento adequado, contribuindo para a redução da morbidade e mortalidade associados ao melanoma de conjuntiva. OBJETIVO: Revisão das características clinicas, diagnóstico e modalidades de tratamento clínico e cirúrgico das lesões precursoras (Nevos e melanose adquirida primária) e do maligno conjuntival. MÉTODOS: Revisão de literatura através de pesquisa no banco de dados MEDLINE, PUBMED, LILACS e SciELO no período de 1980 a 2011. As palavras-chave utilizadas, individualmente ou em conjunto, foram: "conjunctival melanoma", "primary acquired melanosis", "nevi", "treatment", "chemotherapy", "recurrence", "metastasis" e "mortality". RESULTADO: Características clínicas que permitem o diagnóstico do melanoma conjuntival e sua diferenciação de outras lesões pigmentadas conjuntivais foram consideradas. O estadiamento clínico e patológico, assim como modalidades de tratamento para o melanoma maligno foram revisadas. CONCLUSÕES: Pacientes portadores de lesões pigmentadas conjuntivais devem ser avaliados por um oftalmologista especialista. A história oftalmológica, a história familiar de melanoma, e características clínicas da lesão necessitam de cuidadosa avaliação, incluindo-se a determinação do risco de malignidade. A documentação fotográfica deve ser realizada. O tratamento clínico e planejamento cirúrgico devem ser baseados na suspeita clinica. A análise histopatológica por patologista experiente é fundamental para orientação do tratamento e para identificação de fatores prognósticos, principalmente em casos de doença maligna.


INTRODUCTION: Melanocytic tumors comprise a wide range of lesions. These lesions can be nevi, primary acquired melanosis with or without atypia or malignant as conjunctival melanoma. Early recognition of the clinical features allows precise diagnosis and adequate intervention, reducing the rates of metastatic disease and mortality associated with conjunctival melanoma. PURPOSE: To review clinical features, diagnosis, clinical and surgical treatment modalities of the different precursor pigmented lesions (Nevi and primary acquired melanosis) and malignant melanoma. METHODS: Literature review was performed accessing the database of MEDLINE, PUBMED, LILACS e SciELO between 1980 and 2011. The keywords used, individually or combined, were: conjunctival melanoma, primary acquired melanosis, nevi, treatment, chemotherapy, recurrence, metastasis e mortality. RESULTS: Relevant clinical information for the diagnosis of malignant melanoma and differentiation from other pigmented conjunctival lesions were accessed. Clinical and pathological features and different modalities of treatment for malignant melanoma have been reviewed. CONCLUSIONS: Patients presenting with a pigmented conjunctival lesion should be evaluated by an experienced ophthalmologist. Ocular history, family history of melanoma, and clinical features of the lesion need careful assessment and malignancy risk evaluation. Digital photo documentation should be performed. Clinical or surgical planning should be done based on the clinical suspicion. Careful histopathology analysis is required for treatment orientation and prognostic factors identification, mainly in cases of malignancy.

ABSTRACT

Portador de carcinoma espinocelular da conjuntiva teve a lesão removida, com recorrência em outra localização. O paciente recebeu ciclos de 5-Fluoruracila como tratamento adjuvante à remoção cirúrgica, apresentando evolução desfavorável que chegou à exenteração orbitária. São feitos comentários quanto ao uso de antimitóticos no tratamento destas lesões.


A case of a squamous cell carcinoma of the conjunctiva is presented. The lesion was removed with recurrence in another conjunctival site. The patient received cycles of 5-Fluorouracil as adjuvant treatment to the surgical removal and presented unfavorable evolution, requiring orbital exenteration. Comments are made on the use of antimitotic drugs in the management of these lesions.

ABSTRACT

O estudo prospectivo de 20 pacientes com neoplasias epiteliais da conjuntiva ocular, variando de displasia a carcinoma espinocelular, submetidos a uma biópsia excisional complementada com crioterapia e ceratectomia superficial com etanol 100 por cento, revelou uma taxa de recidiva do tumor de 10 por cento (2 pacientes). O período de seguimento pós-operatório foi em média de 16 meses. A complementaçäo da exérese com crioterapia e ceratectomia reduziu a taxa de recidiva dos tumores de 50 por cento para 10 por cento
